Full School Holiday Program Waiver – I Agree
By booking into our school holiday program, I understand and agree to the following, and I agree to these terms on behalf of my child:
Risk, Liability & Medical Authority

Risk & Liability Waiver — I acknowledge that gymnastics and physical activity involve inherent risks. I accept these risks on behalf of my child.

Emergency Medical Authority — If it is impracticable to contact me, I authorise staff to arrange medical or surgical treatment as needed. I agree to pay all associated costs, including ambulance transport.

Minor First Aid Permission — I consent to staff administering basic first aid (ice, band‑aids, rest) without needing to contact me first.
Behaviour & Participation Requirements

Behaviour & Participation Policy — My child must be able to participate independently for the full program day and manage their own toileting needs.

My child must follow instructions and behave appropriately at all times.

If my child’s behaviour disrupts the session, affects safety or enjoyment, or requires ongoing one‑on‑one attention, they will be removed from the program.

I will collect my child immediately if contacted. No refunds or credits will be provided in these circumstances.
Medical Information Requirements

Medical Information Disclosure — I will provide accurate, up‑to‑date information about my child’s medical, behavioural, physical, or emotional needs, including action plans, injuries, conditions, or factors affecting participation or safety.

I agree to update Medowie Gymnastics immediately if anything changes.

Contagious Illness Policy — My child will not attend if unwell or showing symptoms of contagious illness. Staff may require my child to be collected if they become unwell during the program.
Supervision & Parent Contact

Supervision Disclaimer — I understand that staff provide appropriate supervision but cannot offer one‑on‑one supervision except in emergencies.

Parent Contact Availability — I will remain contactable throughout the program day and able to collect my child promptly if required.
Uniform & Safety Requirements

Uniform & Safety Policy — My child must wear safe, appropriate attire: leotards or fitted activewear; bare feet (socks for trampoline only).

Unsafe clothing (dresses, skirts, loose tops, hoodies, jeans, stockings, tassels, sequins) is not permitted.

Only small stud earrings are allowed. No hoops, necklaces, bracelets, rings, watches, or Fitbits.

Long hair must be tied up; very long hair must be braided or plaited.

My child may be unable to participate until unsafe items are removed or correct attire is provided.
Food, Allergies & Personal Items

Food & Allergy Responsibility — I am responsible for providing safe food for my child. I understand the gym cannot guarantee an allergen‑free environment.

No Nut / High‑Risk Food Policy — If applicable, I agree not to send nut‑based or high‑risk foods.

Personal Belongings Policy — Medowie Gymnastics is not responsible for loss or damage to personal items.

Electronics Policy — Phones, smart watches, and devices must remain in bags and are brought at the child’s own risk.
Photography & Privacy

Photo & Video Consent — I consent to my child being included in group photos or videos for internal use, safety documentation, or promotional purposes unless I notify the club in writing beforehand.

Parent Photography Restrictions — I understand that parents are not permitted to take photos or videos inside the gym to protect the privacy of all children.
Drop‑off, Pick‑up & Late Collection

Drop‑off & Pick‑up Responsibility — Responsibility for my child transfers to staff only once they are signed in and returns to me once they are signed out.

Late Pick‑up Policy — I agree to collect my child promptly. Repeated late pick‑ups may result in additional fees or exclusion from future programs.
Program Changes & Weather

Program Changes Disclaimer — Activities may change at any time for safety, staffing, or operational reasons.

Weather & Heat Policy — Staff may modify activities or relocate children during extreme heat or adverse weather.
Cancellation & Refund Policy

Parent‑Initiated Cancellations — All bookings are final. No refunds are provided for cancellations.

Credits or Change of Day — A credit or change of day within the same holiday program may be offered for unforeseen circumstances, subject to availability.

Illness Cancellations — Credits or changes of day may be offered for illness within the same program only. Refunds are not provided.

Program Cancellations by Medowie Gymnastics — If the club cancels a session, a credit, reschedule, or refund will be offered.

Non‑Attendance — Non‑attendance forfeits the booking with no refund or credit.
